Pelvic Floor Physical Therapy in Katy, TX

Leaking, pelvic pain, painful intimacy and postpartum changes often come down to the pelvic floor muscles. Targeted physical therapy can help them work the way they should.

Pelvic floor physical therapy is a specialized form of physical therapy that evaluates and treats the muscles, nerves and connective tissue that support the bladder, bowel and reproductive organs. It is a first-line treatment for many types of incontinence, pelvic pain and pain with intercourse. At our Katy office, licensed physical therapist Sonya Asghari works closely with Dr. Solafa Elshatanoufy to coordinate your care.

What Is the Pelvic Floor?

The pelvic floor is a group of muscles that stretches like a hammock across the bottom of the pelvis. These muscles support the bladder, uterus and bowel, help control urine and stool, and play a role in sexual function and core stability.

Like any muscles, they can become weak, overly tight or poorly coordinated. Pregnancy, childbirth, menopause, chronic straining, heavy lifting, surgery and stress can all affect them. Weak muscles may lead to leakage or prolapse, while tight muscles can cause pain, urgency and difficulty emptying.

Pelvic floor therapy is not just Kegel exercises. Many women actually need to learn to relax these muscles, and a therapist can tell which approach is right for you.

Conditions Pelvic Floor Therapy Can Help

Therapy is often used on its own or together with medical treatment.

Bladder and bowel

Pain, intimacy and life stages

When Should You Consider Pelvic Floor Therapy?

  • You leak urine with activity or feel sudden urges you cannot hold
  • Intercourse, tampon use or pelvic exams are painful
  • You have pelvic, hip or tailbone pain that has not resolved
  • You are recovering from childbirth and something does not feel right
  • You have been diagnosed with prolapse and want to avoid or delay surgery
  • You are preparing for or recovering from pelvic surgery

See a doctor first if you have new pelvic pain with fever, bleeding after menopause, blood in the urine, or sudden loss of bladder or bowel control. These need medical evaluation. Call 911 in an emergency.

Working in the same practice as Dr. Elshatanoufy makes it easy to coordinate your medical and therapy care.

Your First Visit

Your first appointment focuses on understanding your symptoms and goals. You are always in control, and every part of the assessment is explained before it happens.

  • Conversation about your history: symptoms, pregnancies, surgeries, bladder and bowel habits, and what you hope to achieve.
  • Posture, breathing and movement assessment: including the hips, back and abdominal muscles.
  • Pelvic floor muscle assessment: which may include a gentle external or internal exam, only with your consent.
  • A personalized plan: with exercises to start at home.

What Treatment Involves

Every plan is individualized. Treatment usually combines hands-on care in the clinic with a home program.

In-clinic therapy

  • Manual therapy: to release tight muscles, trigger points and scar tissue.
  • Pelvic floor muscle training: to strengthen, relax or coordinate the muscles as needed.
  • Neuromuscular re-education: to improve muscle control and timing.
  • Therapeutic exercise: for the core, hips and posture.

Skills for daily life

  • Bladder retraining: urge suppression and gradually lengthening time between bathroom trips.
  • Bowel retraining: positioning and techniques to empty without straining.
  • Dilator guidance: for vaginismus and painful intercourse.
  • Education: safe lifting, return to exercise and protecting the pelvic floor long term.

Pelvic floor therapy is often combined with medical treatment from Dr. Elshatanoufy, such as vaginal estrogen, bladder medication or a pessary.

What Can You Expect?

Most patients attend sessions over several weeks to a few months, along with a short daily home program. Many notice changes within the first several weeks. Progress depends on your condition and on consistent practice at home. Results vary, and your plan is updated as you improve.

I can’t say enough good things about Dr. Solafa. She is warm, attentive, and incredibly compassionate. She takes her time during appointments, listens carefully to concerns, and never makes you feel rushed. Her bedside manner is truly wonderful, and she creates such a comfortable and reassuring environment. You can tell she genuinely cares about her patients. Very sweet, professional, and knowledgeable. I’m so grateful to have found her for my mom’s treatment.

Dr. Elshatanoufy was truly lovely to work with. I’ve had a hard time pinning down my symptoms but she was willing to hear my concerns and do a thorough examination to rule out more serious conditions. She was thorough, efficient, patient, gentle, and kind during the pelvic examination, which is a vulnerable position for women. I felt that she genuinely cared for my wellbeing. After receiving less than ideal care at a neighboring practice in Katy, I am really grateful to Dr. E for validating my concerns, making me feel heard, and reassured with my next steps.

The waiting room was pretty busy and cold when I went. Suggest bringing headphones and some long sleeves!

My experience perfection. From check in to check out. Dia was my MA and she was so professional and a pleasure. She answered all my questions and made sure I was taken care of. Dr Elshatanoufy was AWESOME! Great bedside manner and make my exam comfortable. Then I had the pleasure to check out with Elizabeth and she was just as pleasant. I can tell this is an office that cares greatly for their patients!

It was a great experience, from checking in with a very helpful and friendly receptionist to being seeing by Nida the office manager, Oyin the NP and Dr. E. Everyone was very friendly, professional, and made the visit very comfortable. They thoroughly explained all the finding and the options for treatment. I highly recommend this practice.
