What to bring
- Photo ID and insurance card
- A list of your medications and allergies
- Records or test results from other doctors, if you have them
- Operative reports from any pelvic surgery
- A list of your symptoms and questions

- We care for women 18 and older
- We accept most major insurance plans
- Some plans require a referral, so please check with your insurer
- You may be asked to keep a short bladder diary before some visits

No. Our only office is in Katy at 410 W Grand Pkwy S, Suite 320. Patients from Fulshear, Richmond, Sugar Land, Cypress and Houston are seen there.
